APPLICATION FORM ACCORDING TO ARTICLES 11 AND 13 OF THE PERSONAL DATA PROTECTION LAW
Personal data owners defined as relevant persons in Law No. 6698 on the Protection of Personal Data (hereinafter referred to as "Applicant") are granted the right to make certain requests regarding the processing of their personal data under Article 11 of the Law.
Pursuant to the first paragraph of Article 13 of the Law; applications regarding these rights to our company, which is the data controller, must be submitted to us in writing or by other methods determined by the Personal Data Protection Board.
Your application and the request contained therein will be concluded free of charge by YAKE, the data controller, as soon as possible and no later than thirty days, depending on the nature of the request. However, if the process requires an additional cost, a fee may be charged according to the tariff determined by the Board. Your request will be accepted or rejected with justification within this period, and the response will be notified to you in writing or electronically.
The information and documents you provide in the application form will be processed by YAKE solely for the purposes of evaluating your application, responding to it, delivering the response to you, and verifying your identity and address. https://yakestore.com/pages/kvkk It will be processed within the scope of the disclosure text available at the address.

According to Article 7 titled "Fee" of the Communiqué on Procedures and Principles for Application to the Data Controller, "(1) If the response to the relevant person's application is to be given in writing, no fee will be charged for up to ten pages. A processing fee of 1 Turkish Lira may be charged for each page exceeding ten pages. (2) If the response to the application is provided on a recording medium such as a CD or flash drive, the fee requested by the data controller cannot exceed the cost of the recording medium."
